CONSULTATION FOR CHANGE OF OWNERSHIP AND MENU REVIEW.
CHANGE OF OWNERSHIP APPLICATION AND $100 FEE RECEIVED. COSMETIC REMODELING IN PROGRESS. EXISTING HAND SINK AND 3-COMPARTMENT SINK WILL BE USED. COOLER AND ESPRESSO MACHINE WILL BE INSTALLED. WATER INLET AND DRAIN LINE WILL BE INSTALLED. BEFORE OPENING, THE FOOD LICENSE APPLICATION AND $175 PRO-RATED FEE WILL NEED TO BE RETURNED TO KCHD. NO PRE-OPENING INSPECTION WILL BE REQUIRED, BUT AN ORIGINAL INSPECTION WILL OCCUR SHORTLY AFTER OPENING. PLEASE CONTACT KCHD TO LET US KNOW YOUR OPENING DATE. BASED ON OUR DISCUSSION OF MENU AND FOOD HANDLING PROCEDURES, THIS FACILITY WILL BE MEDIUM RISK. |
